Moving from pediatric to adult endocrinology — or starting college — is an exciting milestone, but it also comes with new responsibilities in managing your Type 1 diabetes. At Precision Diabetes & Endocrinology, we specialize in helping young adults navigate this important transition with confidence, independence, and strong support.

Our Transition Program is designed specifically for:

• Teens and young adults graduating from pediatric diabetes care

• Students preparing for or currently attending college

• Young adults seeking a smoother shift to adult endocrinology

Note: We see patients age 16 and up

  • We will become licensed in the state of your college if given advanced notice
